Care Planning and Coordination of Services

A care manager, after becoming familiar with an elder and his family support system through a comprehensive yet conversational assessment,  is prepared to suggest a plan that will best meet the elder’s needs while providing relief for a caring family.

Knowing which services and agencies are best equipped to meet needs, preferences and requirements, helping to coordinate communication so there is a minimum of confusion and duplication is an important contribution to a family plan of care.
